- Ability to navigate electronic medical records and patient database to track patient data and facilitate reporting of patient outcomes related to preventative interventions
- Provide nursing support to the clinic by assisting in daily management of patient flow throughout the continuum of care rendered in the clinic to ensure optimum care delivery.
- Coordinate training of all clinical staff in effectively delivering Partnership for Health (PFH) intervention and support integration of intervention into clinical sessions
- Effectively deliver evidence-based interventions with fidelity around topics of prevention and risk reduction education, treatment adherence, sexual health and substance use prevention.
- Provide ongoing treatment adherence support for patients, emphasizing the importance of adherence to medical appointments and, as appropriate, adherence to antiretroviral therapy and other medications.
- Provide a patient centered, holistic approach to patients with identified issues with adherence in order to achieve and maintain compliance with medical care and treatment.
- Function as a liaison with patients and medical providers and assists in facilitating access to medical and social support services by participating in case conferencing in conjunction with case management staff.
- Facilitate timely follow-up appointments with the member's primary care provider (PCP) and obtains relevant inpatient documentation for PCP review.
- Enter and track patient data for patients receiving PFH intervention
- Perform other appropriate duties and participates in other special projects as assigned.
- Project a positive image in representing and demonstrates effective interpersonal and communication skills.
- Support the mission, philosophy, goals and objectives of Wyckoff Heights Medical Center.
- Other duties related to scope of position as assigned..
- Attend necessary ongoing medical education.
- Provide Health Literacy Assessments and documents appropriately in medical chart.
Position Requirement:
- This position requires a minimum formal education of Associate Degree or certification equivalent and a minimum of 3 years job-related experience.
- HIV Specialist certification
- 1 - 3 Years HIV related experience preferred.
