What We’re Looking For
We are seeking a Patient Intake Coordinator who is detail-oriented, organized, and compassionate. The primary purpose of this position is to:
- Assess all patient referrals and assign them to the appropriate resource, including referrals sent by phone, fax, and email.
- Reach out to referral sources to collect any missing or incomplete data.
- Establish patient needs and verify insurance coverage.
- Accurately enter patient data into our electronic health record (EHR) system.
Home health or medical experience is preferred, but not required—we’re happy to provide training and support. Strong communication skills, accuracy, and the ability to multitask are essential. This role is perfect for someone who thrives on making processes run smoothly and enjoys being the “first step” in a patient’s journey of care.
