Position Summary
Under general supervision, the Infection Preventionist performs, coordinates, and supports infection surveillance, prevention, and control activities within the Skilled Nursing Facility (SNF). The position is responsible for identifying healthcare-associated infections and implementing appropriate interventions to ensure safe resident care and compliance with applicable regulatory requirements.
The Infection Preventionist serves as a resource to clinical staff and participates in performance improvement activities related to infection prevention and control.
Key Responsibilities
Performs ongoing infection surveillance activities to identify healthcare-associated infections and other adverse outcomes.
Collects, reviews, and analyzes clinical and laboratory data to identify trends, risks, and reportable conditions.
Implements and monitors infection prevention and control interventions, including isolation precautions and transmission-based practices.
Assists in the development, implementation, and evaluation of the infection prevention and control program and plan.
Provides education and training to staff on infection prevention practices, policies, and procedures.
Participates in quality assurance and performance improvement activities related to infection prevention.
Collaborates with nursing staff, leadership, and interdisciplinary teams to support compliance with infection control standards.
Ensure compliance with District policies, infection control standards, and applicable state and federal regulations.
Maintains required documentation, logs, and reports related to infection prevention activities.
Supports outbreak investigation, response, and required reporting.
