About the Role:
Provide direct patient care to residents, including activities of daily living (ADLs) such as bathing, dressing, and grooming
Assist with mobility and transfers, as well as providing emotional support and conversation
Collaborate with the interdisciplinary team to develop and implement individualized care plans
Maintain accurate and detailed records of resident care and communicate effectively with other team members
Responsibilities:
Provide hands-on care to residents, including but not limited to:
