The Advanced Practice Paramedic Mobile Integrated Health (MIH) provides proactive care that is intended to help improve the overall health and wellness of community and program participants. The Advanced Practice Paramedic MIH initiates activities to reduce high volume utilizers through the investigation of and coordination with community partners to address chronic patient issues, conducts home visits to reduce hospital admissions as part of transitional care in conjunction with community partners, encourages and facilitates use of health care services more appropriate to the needs of the patient than the hospital emergency department, particularly for those patients who are in acute crisis from mental health and substance use disorders. The position includes development and implementation of individualized patient care plans and the delivery of care in accordance with Mobile Integrated Health (MIH) directives and physician orders. The role emphasizes Mobile Integrated Health initiatives, including high-utilizer reduction, chronic disease management, and appropriate utilization of healthcare resources. Work is performed under the direct supervision of the Community Paramedic Captain.

- Provides skilled, professional, and compassionate mobile integrated healthcare to Onslow County citizens.

Provision of Care Coordination to include examining, treating, and screening health services for patients.

Conducts post-hospital release follow-up care including, but not limited to medication reconciliation, wound care, vital signs, and medication administration. Able to deploy or trouble shoot patient monitoring equipment in the home.

Observes, records, and reports to physician patient’s conditions and reactions to drugs, treatments, and significant incidents.

Conducts patient education, includes diabetes prevention/treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), falls assessments, injury evaluation, geriatric frailty visits, and nutrition.

Functions as physician and hospital extender through advanced protocol utilization

Formulates and implements individualized patient care plans

Provides patient care in accordance with Mobile Integrated Health (MIH) directives and physician orders

Exercises clinical judgment within established protocols and physician oversight in non-emergent settings

Serves as trained navigator of community resources

Comprehensive knowledge of principles and processes for providing customer services, including setting and meeting quality standards for services and evaluation of customer satisfaction.

Ability to handle a variety of customer service issues with tact and diplomacy in a confidential manner

Ability to communicate ideas effectively, including the preparation of reports and logs. Ability to listen and understand directions, information and ideas presented verbally and in writing

Attends community meetings and provides community trainings, as requested.

Preform other duties as assigned depending on the community need and/or home situation.

Performs assigned functions as a Disaster Ready Team (DRT)

SUPPLEMENTAL INFORMATION High school diploma or GED with coursework in emergency medical services, or related field and considerable EMS experience, at the Paramedic level, in a 911 system or equivalent combination of education and experience.
Special Requirements:

Valid driver's license.

Currently credentialed to function as a Paramedic in the Onslow County-Camp Lejeune EMS System or the ability to gain credential within six months of hire.

Currently credential to perform Drug Assisted Intubation (DAI) in the Onslow County-Camp Lejeune EMS System or the ability to gain credential within 12 months of hire.

Completion of Advanced Practice Paramedic course within 6 months of hire.

Preferred: Community Paramedic/Mobile Integrated Healthcare Training Program

Preferred: Associate degree in Emergency Medical Science
