Care Plan Coordinator/ MDS Coordinator

We are looking for a MDS Coordinator to join our team. In this role, you’ll work closely with residents, families, and our care team to create and guide individualized care plans. This position is key to making sure each resident’s needs and goals are met with compassion, dignity, and respect.
What You’ll Do
- Complete and manage Minimum Data Set (MDS) assessments with accuracy and timeliness.
- Partner with residents, families, and the care team to design personalized care plans.
- Communicate and educate staff to ensure care plans are understood and followed.
- Monitor resident care and make updates as needed to support the best outcomes.
- Keep accurate records and provide updates to residents and families.
- Support and guide staff in delivering excellent care.
- Collaborate with healthcare providers and agencies when needed to coordinate care.
What We’re Looking For
- Bachelor’s degree (required).
- Current and active RN license (preferred) or LPN license with equivalent experience.
- Strong communication skills with residents, families, and care teams.
- A compassionate approach and the ability to handle sensitive matters with professionalism.
- Experience in utilization review or a related field (preferred, but not required).
- Ability to work independently and as part of a team.
- Willingness to work occasional overtime, if needed.
Schedule & Work Environment
- Full-time, Monday through Friday
- Day shift
- In-person position at our Bensenville, IL campus
