Transportation Provider - Job Description
PURPOSE: To provide medical and nonmedical transportation to older adults living in Duluth, Hermantown, Proctor, and Superior.
REPORTS TO: Program Manager
CASUAL EMPLOYMENT DEFINITION: There is no guarantee or commitment from Age Well Arrowhead that work will continue indefinitely;
· Compensation is only for time worked;
· Loaded mileage is reimbursed at the federal rate;
· Casual employees can end employment without advanced notice.
Age Well Milestone Bonus Program: $250, $500, and $1000 annually
POSITION DEMANDS:
· Pick up client at their home and transport them to and from their appointment;
· Availability 2-3 days per week or more
· Excellent communication skills, including verbal and written communication;
· Accompaniment and companionship to appointment location when required
· Team player who understands how individual contributions are essential for the success of the program/agency as a whole and who values professionalism;
· Flexible schedule to accommodate client transportation needs;
· Friendly, patient reliable
· Reliable and safe vehicle with up-to-date auto insurance;
· Driver's license with personal vehicle.
PRINCIPAL RESPONSIBILITIES:
· Provide transportation;
· Companionship;
· Follow scheduled pick-up times;
· Provide safe and reliable vehicle;
· Driving clients as requested;
· Effectively communicate with client;
· Comfortably navigate Duluth and Superior locations;
· Provide appropriate record keeping per position requirements;
· Maintain client records as required by funding source(s), HIPAA and AAAA requirements;
· Perform transportation duties as assigned;
· Log hours using Better Impact app;
· Provide information about each client visit using Better Impact;
· Participate in mandatory meetings and trainings;
· Ability to use transportation scheduling program;
· Report to work on time;
· Communicate any concerns regarding the client or your position immediately to the Program Manager;
· Have a backup plan for unanticipated events that may prevent you from reporting to work.
QUALIFICATIONS:
· High school diploma;
· Ability to pass a background check;
· Use of personal vehicle with up-to-date auto insurance;
· Experience working with older adults;
· Ability to work independently and interdependently;
· Ability to communicate effectively both orally and in writing;
· Ability to be trained in new models of support;
· Ability to utilize personal computer for recordkeeping, reporting, communication
· Ability to articulate and support Age Well mission and vision;
· Valid Drivers License;
· Clean driver record is required
· Clean and reliable vehicle
PERFORMANCE IS SATISFACTORY WHEN: Client satisfaction measures report that the program is responsive to their needs; records are complete, accessible, and organized.
Pay: From $17.00 per hour
