Job Description:
The PT Lead is responsible for coordinating and delivering high‑quality therapeutic and wellness services within a retirement/assisted living community. This role combines clinical skills, program coordination, and relationship-building to support residents’ functional independence, promote the company’s services, and foster strong collaboration with community staff and families.
Qualifications:
- Graduate of an approved physical therapy curriculum.
- Possesses a current license to practice as a physical therapist in Ohio.
Responsibilities:
- Assists the physician in evaluating clients by applying approved diagnostic and functional ability tests.
- Treats clients to relieve pain and/ or develop or restore function to maintain maximum performance.
- Makes appropriate arrangements for outpatient client services for services which cannot be given in the home.
- Records and reports to the physician the client’s reaction to treatment and/ or changes in the client’s condition.
- Instructs clients and their families in the use of prosthetic, orthotic and assistive devices (canes, walkers, crutches, wheelchairs, etc).
- Instructs the family in the client’s total physical therapy program.
- Attends care conferences and Agency meetings as necessary.
- Supervises the physical therapy assistant.
- Prepares an assessment of the client’s condition and periodic evaluations and summaries, develops a care plan and writes clinical/ progress notes and submits them to the Agency weekly.
- Communicates effectively with all those providing care.
- Participates with staff, client and physician in discharge planning activities and completes the physical therapy discharge summary.
Additional Responsibilities:
- Referral Management & Admissions Support
- Gather referral information from internal and external sources and process all incoming therapy leads in a timely, organized manner.
- Collaborate with the building’s marketer to ensure smooth transitions for new residents requiring therapy services and to support census growth.
- Clinical & Wellness Programming
- Plan and conduct at least two group exercises or wellness classes each week, tailored to residents’ abilities and needs.
- Assist in developing and implementing individualized therapy and wellness plans under the direction of a supervising Physical Therapist, in accordance with state and federal regulations and company policies.
- Monitor resident participation and progress; communicate relevant changes to the care team and supervising therapist.
- Meetings, Communication & Care Coordination
- Participate in required meetings, including all-staff meetings and daily/morning stand‑ups, to share updates on resident status, therapy caseload, and programming. (When applicable)
- Facilitate and oversee monthly care team meetings, ensuring relevant disciplines are involved, resident goals are reviewed, and follow-up tasks are clearly assigned and tracked.
- Maintain clear, timely, and accurate documentation and communication regarding therapy services, resident progress, and care plans with the management team at your assigned community/ communities.
- Marketing & Community Engagement
- Attend and help organize marketing and community events within the Assisted Living facility to promote therapy and wellness services.
- Represent the therapy department and company professionally to residents, families, staff, and external partners.
- Relationship Management & Leadership
- Build and maintain strong working relationships with key staff members (e.g., Executive Director, Nursing, Activities, Admissions/Marketing, Therapy team).
- Serve as a primary therapy point of contact in the building, helping to coordinate schedules, address concerns, and support a positive, resident‑centered culture.
- Model excellent customer service and teamwork, supporting orientation and mentoring of new therapy staff as needed.
Organizational Relationship:
- Reports directly to the Director of Rehab & Therapy.
Functional Abilities:
- Must be able to read 12 point type or larger.
- Must be able to communicate verbally and in writing in a manner understood by most persons.
- Must be able to stoop, bend and be able to lift and transfer clients.
- Must be able to travel to client’s residences.
Special requirements:
- Ability to lead group exercise classes and engage residents with varying functional levels.
- Comfortable participating in marketing activities and representing the organization to residents, families, and referral sources.
Job Type: Full-time
Salary: $100,100 per year plus incentive opportunities (minimum 35 visits a week) for full-time employees.
Monthly Bonus: Up to $1,000 a month
