General Purpose:
To ensure quality, accuracy, accessibility, and security in both paper and electronic systems. This department uses various classification systems to code and categorize patient information for insurance reimbursement purposes, for databases and registries, and to maintain patients’ medical and treatment histories.
Essential duties and Responsibilities:
- Maintain master patient index
- Assist physicians with record completion
- Contact physicians regarding incomplete charts
- Scan all medical records as requested
- Ensure that appropriate consents, subpoena and other medical record forms are complete upon request for a copy of medical record; obtain authorization from administration
- Respond to requests for records from Federal, State or County courts, hospitals and physicians after consulting with the administrator
- Assist nursing staff and physicians with death certificates
- Assemble condensed portions of inpatient medical records and maintain them over sixty days in medical record department
- Maintain correspondence log
Qualifications/Requirements:
- High school diploma
- Proficient in Medical terminology
- Data Entry (40-50wpm)
- Operating standard office equipment including pertinent software applications; using a multi-line telecommunication system; and preparing and maintaining accurate records.
- Knowledge of administrative and clerical procedures
- 1-2 Years experience
- Knowledge of customer service principles and practices
- Professional personal presentation
