Wraparound Care Facilitator will utilize the Utilization Management Guidelines to guide service delivery based on the level of care the child or adolescent is. The Wraparound Care Coordination is guided by the wraparound philosophy that emphasizes "empowering families". This is defined through the following value base: Building on strengths to meet needs; One family-one plan; Best fit with culture and preferences; Community bases responsiveness; Increased parent choice and family independence; Care for youth in the context of their families; and never giving up. This position will work with providers in the community to develop responsive and flexible resources that facilitate community -based interventions and supports that correspond with the needs of the families and their children and youth.
This position will be assigned a primary caseload of children requiring a higher level of care. Wraparound Care Facilitator duties include ensuring each family participating receives a family directed Plan of Care and Family Story. Work to create a Child and Family team for each youth and family. Works to coordinate direct rehabilitative services that engage clients in activities' designed to reduce symptoms of and disabilities that result from mental illness and to increase the ability of the individual to function in the community. Position must maintain LPHA credentials to perform certain job duties. Assists with YES waiver clinical eligibility, YES Medicaid eligibility and works to interface with YES providers if the youth and family request and are eligible for YES services. Duties of the position will require frequent interactions with individuals served, their families and other service providers. Require flexible hours in meeting the needs of the children and their families, involves intensive paperwork and will require self directed performance. Travel and the use of personal vehicle are required. The Wraparound Care Facilitator reports directly to the Deputy Director of Behavioral Health Essential Services.

Position Requirements:
- Licensed practitioner of the healing arts
- Required: Bachelor's or advanced degree from an accredited college or university
- Must have and maintain a Valid Drivers License and driving record that is acceptable for insurability through our insurance carrier.
- Must pass background check and drug screening.
- Applicants must reside within 70 mile radius of duty site
