The Physical Therapist is responsible for evaluating residents, developing and implementing treatment plans to improve mobility, relieve pain, increase strength, and restore physical function. The PT works collaboratively with an interdisciplinary team to help residents maintain or regain their highest level of functional independence.
- Perform physical therapy evaluations and re-evaluations using appropriate tests and measures.
- Develop individualized treatment plans based on resident diagnoses, impairments, goals, and functional needs.
- Provide direct physical therapy treatment to residents as prescribed.
- Educate residents, families, and staff on mobility strategies, safety awareness, and use of assistive devices.
- Collaborate with physicians, nurses, and other rehab staff to ensure coordinated care.
- Supervise Physical Therapist Assistants (PTAs) and monitor progress toward goals.
- Document evaluations, treatments, progress notes, and discharge summaries per facility and regulatory guidelines.
- Ensure compliance with all state, federal, and Medicare/Medicaid regulations.
