Job Overview
We’re seeking a compassionate and dedicated Hospice Social Worker to join our interdisciplinary team. In this role, you’ll provide emotional support, resource coordination, and advocacy for patients and families navigating end-of-life care. You’ll be a vital part of ensuring dignity, comfort, and peace during life’s most tender moments
Responsibilities
- Conduct psychosocial assessments for hospice patients and families
- Develop individualized care plans in collaboration with the hospice team
- Provide grief counseling, crisis intervention, and emotional support
- Assist families with advance directives, financial concerns, and community resources
- Facilitate family meetings and end-of-life planning discussions
- Maintain accurate documentation in compliance with Medicare and agency standards
- Participate in interdisciplinary team meetings and contribute to holistic care planning
Qualifications
- Master’s degree in Social Work (MSW) from an accredited program
- Current state licensure (LMSW or LCSW preferred)
- Minimum 1 year of experience in hospice, palliative care, or medical social work
- Strong communication, empathy, and organizational skills
- Valid driver’s license and reliable transportation

Pay: $33.00 - $36.62 per hour
