Hospice Specialists of Louisiana is a compassionate, patient-centered hospice organization dedicated to providing exceptional end-of-life care and support to patients and families. We are seeking a highly organized and clinically knowledgeable Clinical Intake Coordinator to join our growing team.
Position Summary
The Hospice Clinical Intake Coordinator serves as the primary point of contact for all incoming hospice referrals and admissions coordination. This role works closely with hospitals, physicians, case managers, families, and the interdisciplinary team to ensure timely, accurate, and compassionate intake processing and patient admissions.
The ideal candidate is detail-oriented, communicates effectively, thrives in a fast-paced healthcare environment, and understands hospice eligibility and regulatory requirements.
Responsibilities
- Receive, review, and process hospice referrals and admissions
- Coordinate patient admissions with clinical staff and referral sources
- Verify eligibility criteria, insurance benefits, and required documentation
- Communicate with hospitals, nursing facilities, physicians, and families regarding referrals and admission status
- Schedule admission visits and coordinate with field clinicians
- Maintain accurate intake records within EMR systems
- Ensure compliance with Medicare, Medicaid, and hospice regulations
- Monitor referral pipeline and maintain strong referral source relationships
- Assist with census growth initiatives and intake reporting
- Provide compassionate customer service to patients and families during the admission process
Qualifications
- Licensed Nurse (LPN/RN preferred) OR minimum 2 years healthcare intake/coordinator experience
- Hospice or home health experience strongly preferred
- Knowledge of hospice eligibility criteria and Medicare regulations
- Excellent communication and organizational skills
- Strong multitasking and time-management abilities
- Experience with EMR systems and Microsoft Office
- Ability to work independently and collaboratively in a team environment
