POSITION SUMMARY:
Under the direction of the Program Manager of PEP and PrEP, the PrEP Navigator is responsible for conducting outreach and inreach activities, assist with intakes and provide education to potential PEP/PrEP patients seeking services. The PrEP Navigator also assist in the coordination of medical appointments, troubleshoots patient insurance issues; enters data into AIRS, conducts follow up with clients, and provides retention and adherence counseling to PEP/PrEP patients.
DUTIES AND RESPONSIBILITIES:

Assesses needs of patients seeking and accessing PEP and PrEP related services.

Assist providers with identifying patients for PEP/PrEP and assist with intakes/program enrollment, service navigation, education and follow up around PEP and PrEP services

Assists with data collection and data entry for the AIDS Institute Reporting System and CHN’s Electronic Medical Record

Coordinates medical services with clinic staff.

Provides a wide range of community outreach, education, and risk reduction interventions.

Participates in health fairs, tabling and joint health activities with other community agencies.

Participate in trainings and support services.

Maintains strict confidentiality of all patient related information.

Facilitates PEP/PrEP related services for health center patients as appropriate with respect to their confidentiality and privacy.

Compliance with Employee Health Services.

Performs other related duties, as assigned.
CERTIFICATE/LICENSE REQUIRED:
-
High School Diploma or G.E.D. equivalency.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
-
Candidate should be a member of the targeted communities and service group(s) (a peer).

Basic knowledge of HIV prevention initiatives and PEP/PrEP.

Good verbal and written communication skills.

Knowledge of geographic area of work site and of language appropriate to community.

Proficient in Microsoft Word and Excel and data entry in AIRS

Respect for the rights of individuals to make their own decisions about their medical care

Ability to communicate easily and display a cordial manner towards individuals from a variety of socio-economic, cultural and religious background.

Bilingual desired.
