Position Overview
The Physical Therapist will evaluate, develop, and implement individualized treatment plans for patients recovering from injury, illness, surgery, or managing chronic conditions. The PT will collaborate with physicians, nurses, caregivers, and interdisciplinary team members to deliver high-quality, patient-centered care in a home health setting.
Responsibilities

Perform comprehensive physical therapy evaluations and assessments.

Develop and implement individualized treatment plans based on patient needs and goals.

Provide therapeutic interventions to improve mobility, strength, balance, coordination, and functional independence.

Educate patients and caregivers on treatment plans, home exercise programs, and injury prevention.

Accurately document evaluations, treatments, progress notes, and discharge summaries in accordance with regulatory and agency requirements.

Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.

Monitor patient progress and modify treatment plans as needed.

Ensure compliance with Medicare, Medicaid, state regulations, and company policies.
Qualifications

Doctor of Physical Therapy (DPT) or equivalent degree from a CAPTE-accredited physical therapy program.

Current Iowa Physical Therapist License issued by the Iowa Board of Physical and Occupational Therapy.

Current CPR/BLS certification or ability to obtain prior to start.

Valid driver's license, reliable transportation, and current auto insurance.

Ability to travel within assigned service areas.

Strong communication, documentation, and interpersonal skills.

Previous home health experience preferred but not required.
