A Physicians Home Care (APHC) is a locally owned business in Colorado Springs that was started by a family of a Special Needs child who is now an adult.
We founded in 2017 in hopes to be able to provide the best possible care to those who need in home assistance. Our motto is where everyone is treated as family and our main goals are to give back and help others. We are licensed by the state of Colorado to provide In Home Support Services (IHSS) as well as Accredited by the state and ACHC to provide CNA, Skilled Nursing, and Therapy Services.
We are seeking a Physical Therapist that has knowledge of physical therapy practice, with emphasis on the provision of physical therapy services in a home environment to provide compassionate and quality care.
Pay: Up to $165,000/year

Responsibilities/ Abilities
* Complete comprehensive and OASIS assessments/ visits on applicable client's
* Perform Physical Therapy (PT) duties as appropriate and generate Home Exercise Plans for our families
* Supervise Physical Therapy Assistants (PTAs) as assigned
* Ensure the quality of services is maintained
* Collaborate with the other members of A Physicians Home Care staff
* Maintain HIPAA privacy practices
* Ability to foster constructive and therapeutic relationships with patients/clients and their families
* Ability to communicate effectively, both orally and in writing.
* Other Physical Therapy (PT) duties as assigned.
Requirements
* Graduate of an accredited physical therapy curriculum/school approved by the American Physical Therapy Association or The Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association.
* Current and Valid Physical Therapy License for State of Colorado
* CPR certification
* Current, Valid Driver’s license
* Access to a reliable automobile
