JOB DESCRIPTION: The primary duties of the RN include timely phone triage, care coordination calls, and providing clinical support to the physicians, providers, and practice staff. Administration of very occasional Aranesp and Retacrit, IV fluids, immunizations and provide patient care under the direction of medical doctor or mid-level providers are also included. Assist on prior authorizations.
JOB RESPONIBILITIES:
- Ensure all Telephone Encounters are responded to efficiently and rapidly as received from providers and practice staff following appropriate HIPAA guidelines for patient confidentiality and concise documentation in EMR to reflect current status. Prior to end of shift in evening or Fridays, review and confirm all high priority patient calls have been addressed per MD established protocols.
- Timely completion and documentation of TCM outreach calls per CMS requirements within two days of the patient’s discharge from a hospital or facility as appropriate per MD.
- Support practice’s Remote Patient Monitoring Program for blood pressures, pulse and weight management via patient phone calls and ECW Telephone Encounters.
- Assist female patients when assistance is needed in patient bathrooms
- Assist in medical emergency situations that may arise in the lab, office and patient areas involving patients, staff, and visitors
- As needed, assist on prior authorizations, scheduling of blood transfusions, infusions, and other complex testing per MD Orders.
- Support patient education and care coordination efforts of the practice including but not limited to transitional care services and chronic care management services, and other programs as needed.
- Provide support for clinic and respond quickly and collaboratively to provider and staff requests.
- Occasionally, provide Administration of Aranesp and Retacrit per office protocol.
- Occasionally, provide Administration of flu, hepatitis vaccinations.
- Provide completion of annual PPD tests for office staff including placement and reading.
- As assigned, perform annual PPD tests for office staff including placement and reading
- Complete documentation for nurse visits, Aranesp, Retacrit, I/V Fluid, Magnesium, flu, hepatitis B, pneumonia, and PPD visits.
- Record and maintain concise and clinically appropriate documentation in EHR in timely fashion; chart notes to be completed same day or 1-2 business days
- Perform venipuncture for iron studies and CBC
- Assist in maintaining refrigerator temperature log
- Assist in ordering and reconciliation of clinical supplies
- Other duties as requested
JOB QUALIFICATIONS:
- High school diploma
- Current NYS RN license
- Excellent communication and patient care skills
- Proven record of accomplishment of consistent attendance and high degree of teamwork and collaboration.
- Ensure compliance with all HIPAA, Privacy and Security requirements of the practice.
- CPR certification
SECURITY:
- Protecting the practice’s hardware, software information, and data by complying with its security policies and procedures and correctly using technologies
- Attend required security training
- Use computer terminals and workstations in a manner that protect the information they access
- Maintain the confidentiality passwords and patient health information
- Report all security breaches to Clinical Manager.
Pay: $25.00 per hour
