Job Summary:
True Family Services is seeking a compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) for our overnight shift. This position is ideal for a retired nurse or someone looking for a low-stress, supplemental role while continuing to make a meaningful impact.
This role is primarily focused on intake nursing assessments and coordination of client medical information within the crisis services team.

Duties
- Complete nursing assessments during client admissions/intake
- Obtain and document vital signs, medical history, medications, allergies, and presenting concerns
- Observe clients for medical or behavioral concerns and report findings appropriately
- Coordinate with nursing staff, providers, QMHPs, clinicians, and leadership regarding client care needs
- Maintain accurate and timely documentation in the electronic health record
- Assist with medical clearances and communication with hospitals or outside providers as needed
- Monitor clients for changes in physical condition while on the unit
- Ensure compliance with DBHDS, DMAS, CARF, and agency policies and procedures
- Participate in shift reports and nursing communication updates
- Maintain confidentiality and professionalism at all times
Requirements
- Valid LPN license in the state of practice.
- Proven experience in home care or outpatient settings is preferred.
- Strong skills in medication administration and venipuncture techniques.
- Knowledge of ICD-9 coding standards is a plus.
- Experience providing care for crisis clients and individuals with psychosis is highly desirable.
- Excellent communication skills, both verbal and written, with a focus on patient education.
- Ability to work collaboratively within a healthcare team while maintaining high standards of patient care.
- Strong organizational skills with attention to detail in documentation and reporting.
Qualifications
- Active Virginia LPN license in good standing
- Current CPR certification
- Experience in behavioral health, crisis services, psychiatric care, emergency services, or substance abuse preferred
- Strong communication and documentation skills
- Ability to work in a fast-paced crisis environment
- Ability to work collaboratively within a multidisciplinary team
