Job Summary
We are seeking a compassionate and dedicated Caregiver to provide exceptional support to clients enrolled in the Pathways Waiver program. This vital role involves delivering personalized care that promotes independence, safety, and well-being for individuals with complex needs, including developmental disabilities, neurodevelopmental disorders, and aging-related conditions. Duties
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patients’ vital signs and observe for any changes in health status, documenting findings accurately in care plans
- Administer medications following HIPAA guidelines and prescribed instructions, ensuring safety and compliance at all times
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed
- Support clients with memory care needs, including dementia and Alzheimer’s care, by implementing personalized strategies to ensure safety and comfort
- Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces
- Follow established care plans meticulously while providing direct support that respects each client’s dignity and preferences
- Use manual handling techniques safely during transfers or repositioning, employing tools like Hoyer lifts when appropriate
- Observe patient behavior for signs of distress or changes in condition; communicate effectively with healthcare teams regarding patient status
- Support clients with neurodevelopmental disabilities or autism by implementing tailored activities that promote engagement and development
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred
- Demonstrated ability to monitor patients’ vital signs and administer medications accurately while adhering to HIPAA regulations
- Hands-on experience with dementia care, Alzheimer’s care, memory care or hospice environments is advantageous
- Proven skills in assisting with ADLs, manual handling techniques, and using mobility aids such as Hoyer lifts
- Familiarity with developing or following care plans for individuals with developmental disabilities or neurodevelopmental disorders
- Experience working with diverse populations including seniors, individuals with intellectual disabilities or autism spectrum disorder is valued
- Knowledge of safety protocols related to patient monitoring, first aid procedures including CPR, heavy lifting techniques, and manual handling practices
