This position is 37.50 hours per week. Monday through Friday, day time hours. No on-call.

The LPN is responsible to ensure and advocate for quality medical care for all persons’ served in residential services, assist with any medical emergencies in day services and provide medical-related training.
Major Responsibilities and Tasks: Supervises the health needs of all individuals in the assigned CLA(s)Reviews results of medical appointments and signs off on sheets; Obtains specialty evaluations (OT, PT, etc) as needed. Reviews and writes summary reports for all IP meetings (except Nursing Interventions, the “care plan,” which is to be written by LPN. Conducts follow up with doctors, families and others as appropriate. Edit (new unsigned) Physician Order Sheets as changes occur an sign off as person editing prior to sending to be signed by MDCompletes running notes on clients and develops care plans for clientsCompletes house audits of medical recordsReview monthly kardex’s before the start of the month (as 1st check whenever possible).Review kardex’s at least weeklyCompletes monthly pharmacy reviews based on kardex reviewReviews Medical Consent Forms and ensure they are current and updated as neededEnsures all medical appointments are scheduled and completed according to physician’s recommendations and DDS regulations.Schedules specialty exams; obtains results of testing (e.g., echocardiograms, x-rays, etc.)Ensures updated information is communicated to day program as neededServes as primary contact with external nursing agencies/OTs/PTs in assigned programs.Maintains contact with local health care providersAttends IP Meetings as necessaryCarries out systems to assist in preparation for all external inspections and auditsReviews medical supplies for housesChecks medications located at each house for expiration and proper labelingCompletes inventory review of all items belonging to medical departmentMaintains report on medication errors and provides appropriate follow-up action to both client and staff when errors occurWhen trained to do so, completes all annual medication recertification’s for staff in primary program locations and as a backup in alternate locations.
