REGISTERED NURSE – BEACON HOME HEALTH

POSITION SUMMARY: The Registered Nurse plans, organizes, directs and coordinates home care services. The RN demonstrates experience in nursing, with an emphasis on community health education. The professional nurse utilizes community resources to provide services that meet the needs of patients, families, and caregivers within their homes and communities.
ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

Develops and initiates the plan of care and assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patients, families, and caregivers.

Initiates proper preventative and rehabilitative nursing procedures and provides services that are ordered by the physician as indicated in the plan of care.

Provides patient, family, and caregiver counseling.

Provides patient, family, and caregiver education.

Prepares clinical notes.

Communicates with the physician who is responsible for the home health plan of care and other health care practitioners, as appropriate, related to the current home health plan of care.

Identifies discharge planning needs as part of the care plan development and implements prior to patient discharge.

Prepares clinical notes and updates the primary physician when necessary and at least every sixty (60) days.

Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.

Communicates with community health-related persons to coordinate the care plan.

Participates in the agency’s quality assessment and performance improvement program and agency-sponsored in-service trainings.

Participates in on-call duties as defined by the agency policy.

Ensures that arrangements for equipment and other necessary items and services are available.

Instructs, supervises, and evaluates Home Health Aide care provided every two (2) weeks.

Supervises Licensed Practical Nurses (LPNs)/ Licensed Vocational Nurses (LVNs) and/or Home Health Aides (HHAs)/Certified Nursing Assistants (CNAs), as directed.

Other appropriate services and special projects as assigned.

QUALIFICATIONS & REQUIREMENTS FOR THE POSITION:

Current RN licensure in state of practice.

At least two (2) years of experience working as a Registered Nurse in health care, home health experience preferred.
- Current CPR certification required.

Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
PREFERRED KNOWLEDGE AND SKILLS:
- Excellent time management and problem-solving skills.

Must have outstanding verbal and written communication skills.

Must be self-motivated and able to work independently while being collaborative and able to work as part of an interdisciplinary team.
SUPERVISORY RESPONSIBILITIES:

May supervise HHA, CNA, LPN, and/or LVN as needed.

Must have the reasonable ability to stand, walk, sit, use hands to finger, handle, feel, reach, balance, see, talk, hear, and drive. Must be reasonably capable of lifting over thirty (30) pounds.

Significant portion (>50%) of daily assignments may require travel to client/resident/patient locations or other work sites, via car or public transportation.

Work schedule may occasionally require evening, night, holiday, or weekend assignments.
