About the Position
- RN Field Case Managers are responsible for comprehensive assessments in the patient’s home, including the completion of OASIS using electronic documentation.
- In addition to OASIS assessments, RN Field Case Managers perform skilled nursing visits as needed and are responsible for delegation and coordination of care with LVN and HHA team members
- Must be fully skilled in lab draws, catheter placement, wound care , wound vac and IV
Requirements
- Current Texas Registered Nurse license
- Current Texas driver's license
- Automobile liability insurance issued in Texas
- Minimum 2 Years of Home Health nursing experience.
- Minimum of 1 yr living in the DFW area.
Job Type: Full-time
Pay: $90,000.00 - $110,000.00 per year
