JOB SUMMARY:
The occupational therapist employed by the Agency is responsible for the implementation of standardsof care for occupational therapy services and for the adherence to all conditions outlined in their scope of practice.
JOB QUALIFICATIONS:

1. Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
2. Registered with the National Registration of the American Occupational Therapy Association.
3. Two years of clinical experience as an occupational therapist.
4. Community/home health experience is preferred.
5. Possesses and maintains current CPR and BLS certification.
6. Must have valid Florida driver's license, a reliable automobile and minimum insurance as required by law.
7. Licensed to practice as occupational therapist within the State.

The physical therapist contracted or employed through the Agency is responsible for providing physical therapy services to promote positive therapeutic outcomes and for adherence to all conditions outlined in the Personnel Services Agreement.

1. Graduate of a physical therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association.
2. Possesses a degree from a baccalaureate, master’s, or doctorate program in physical therapy approved by an accredited organization.
3. Minimum of two years of appropriate experience as a physical therapist.
4. Community/home health experience is preferred.
5. Possesses and maintains current CPR certification and BLS Certification.
6. Must have valid Florida Driver's license, a reliable automobile and minimum insurance as required by law.
7. Licensed to practice as physical therapist within the State.
8. Must possess a caring and compassionate demeanor
