- What You’ll Do (The Day-to-Day)
As a TLC, you will be the ultimate coach, helping individuals learn habilitation and recovery skills, connect with community resources, and crush their personal goals.
Your main responsibilities will include:
- Building Trust: Engage and develop positive, life-changing relationships with residents.
- Teamwork: Partner with the broader service team to coordinate great outcomes.
- The Essentials: Perform assessments and develop personalized service plans.
- Skill Building: Provide restorative services that help people regain independence.
- Documentation: Keep timely, accurate records in line with NYSOMH and Medicaid rules.
Note: You will manage a small, focused caseload of just 8-9 cases and participate in a periodic, rotating weekly on-call schedule.
✅ Who You Are (Qualifications)
We value diverse backgrounds! You fit the bill if you have one of the following:
- A High School diploma + 5 years of experience in human services.
- An Associate’s degree in human services + 3 years of experience.
- A Bachelor’s degree in a human services related field.
