Job Duties
- Provide skilled nursing care to approximately 6 patients per day in their homes, following individualized care plans and established clinical protocols.
- Deliver patient care under the direction of the RN Case Manager and/or Clinical Team Manager in accordance with the State Nurse Practice Act and professional standards.
- Perform comprehensive nursing assessments, monitor patient conditions, and report changes in health status to the RN Case Manager or Clinical Team Manager.
- Administer medications and treatments as ordered and within the scope of the LPN license.
- Perform wound care, dressing changes, catheter care, vital signs, and other skilled nursing procedures as required.
- Educate patients and caregivers regarding medications, treatment plans, disease management, safety, and proper home care techniques.
- Collaborate with RNs, Physical Therapists (PTs), Occupational Therapists (OTs), HHAs, and other members of the interdisciplinary care team to support patient goals.
- Document patient visits, assessments, interventions, and outcomes accurately and in a timely manner.
- Recognize changes in patient condition and promptly communicate concerns to the RN Case Manager and Clinical Team Manager
Requirements:
- 1+ Year of Home Health Experience required
- MA License Required
- BLS

Pay: $1,600.00 - $1,900.00 per week
