Job Overview
We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. In this vital role, you will assess, plan, and implement personalized rehabilitation programs to help patients regain mobility, reduce pain, and improve overall quality of life. Whether working in inpatient hospital settings, outpatient clinics, or home health environments, you will be instrumental in delivering comprehensive patient care that promotes recovery and independence. Responsibilities
- Conduct thorough evaluations of patients’ physical conditions, including mobility, strength, and functional abilities
- Develop and implement individualized care plans tailored to each patient’s needs and recovery goals
- Administer physiotherapy treatments such as trigger point therapy, manual therapy, and therapeutic exercises
- Monitor patient progress regularly and adjust treatment strategies accordingly to optimize recovery outcomes
- Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transition of care in hospital or community settings
- Educate patients and their families on exercises, injury prevention, and home care routines to support ongoing recovery
- Maintain accurate documentation of treatment sessions, progress notes, and care plans using electronic medical record (EMR) systems
Requirements
- Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction
- Proven experience in acute care, inpatient hospital settings, outpatient clinics, or home health environments
- Strong knowledge of physiology, anatomy, medical terminology, and orthopedic principles
- Experience with care planning for diverse populations including pediatrics and geriatrics
- Familiarity with EMR systems for documentation and patient record management
- Ability to perform discharge planning effectively and coordinate with healthcare teams for seamless patient transitions
- Excellent communication skills to educate patients on physiotherapy techniques and home & community care strategies

Pay: $40.00 - $50.00 per hour
