Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
- Monitor and document patient observations, vital signs, and medical conditions accurately using Electronic Health Record (EHR) systems while maintaining strict adherence to HIPAA regulations to ensure patient confidentiality.
- Support medication administration under supervision, including medication reminders and basic medical documentation, following home healthcare regulations.
- Provide specialized care for patients with dementia, Alzheimer’s disease, or other memory impairments through tailored memory care techniques and compassionate engagement.
- Assist with patients with care needs with sensitivity and professionalism.
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment conducive to patient health and comfort.
- Support patient transfer methods safely using proper techniques and equipment like Hoyer lifts or patient transfer belts while adhering to safety protocols.
- Assist with appointments & attending appointments with patients
Requirements
- Proven experience working with individuals with disabilities, geriatrics, or in assisted living environments; experience in nursing homes or long-term care settings is highly valued.
- Knowledge of medical terminology, vital signs measurement, medication administration, catheter care, and basic life support (BLS) including CPR.
- Familiarity with EMR/EHR systems for accurate medical documentation; experience working with home health agencies or hospice providers is a plus.
- Ability to perform heavy lifting when necessary and maintain physical stamina for long shifts involving patient transfers and caregiving tasks.
- Strong communication skills to effectively interact with patients, families, and healthcare teams while maintaining patient confidentiality per HIPAA standards.
- Certification as a CNA or equivalent credential required; additional certifications such as First Aid are advantageous.
- Understanding of home health care regulations and best practices for providing compassionate client-centered care in diverse settings.
