Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices
- Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; document findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
- Medication reminders following prescribed protocols while maintaining strict adherence to HIPAA regulations to ensure patient confidentiality
- Support clients with specialized needs such as Alzheimer's care, dementia care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities
- Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients
- Maintain detailed medical documentation and records of patient care activities in compliance with healthcare standards
- Collaborate with healthcare teams to coordinate patient care plans and communicate changes or concerns effectively
Experience
- Prior experience in home health care or assisted living environments preferred; experience in nursing homes or long-term care settings is highly valued
- Demonstrated ability to work with individuals with disabilities or complex medical conditions such as dementia or Alzheimer's disease
- Familiarity with medical terminology and medical documentation practices
- Certification in CPR and first aid required; additional certifications such as catheter care or hospice care are advantageous
- Experience operating EMR/EHR systems for accurate record keeping and communication
- Proven ability to handle sensitive situations with compassion while maintaining professionalism and confidentiality
- Strong understanding of patient observation techniques and medication administration protocols
