Title: RN Case Manager
Location: Minneapolis & Saint Paul Area, Minnesota
Hours: Monday - Friday, 8:00 AM - 5:00 PM
(Occasional weekend rotation required)
Pay: $45.00/hour
RESPONSIBILITIES
- Conduct 6-8 in-home patient assessments daily within assigned territory
- Complete OASIS assessments including Start of Care (SOC), Resumption of Care (ROC), and Recertifications
- Develop, implement, and update individualized patient care plans
- Coordinate interdisciplinary services including PT, OT, ST, and other supportive care services
- Provide skilled nursing care and patient/family education
- Supervise LPNs, CNAs, PCAs, and HHAs to ensure quality patient care
- Document assessments, visits, and care plans accurately within EMR systems
- Communicate patient condition changes to physicians and clinical leadership
- Participate in care conferences and interdisciplinary team meetings
- Ensure compliance with CMS, Joint Commission, and state/federal regulations
REQUIRED QUALIFICATIONS
- Active Minnesota RN License in good standing
- Minimum 6 months to 1 year of OASIS assessment experience
- At least 1 year of Home Health RN Case Management experience
- Graduate of an accredited nursing program (BSN preferred)
- Current BLS/CPR certification (AHA preferred)
- Valid driver's license, reliable transportation, and active auto insurance
- Strong clinical assessment and critical thinking skills
- Experience with EMR documentation systems
- Ability to manage an independent caseload effectively
- Ability to pass background check, drug screen, and driving record review
