Job Description:
Hillstone Law is seeking an experienced Lien Negotiator to join our growing personal injury team. This role is responsible for reviewing, verifying, and negotiating medical liens and outstanding balances to maximize clients’ net settlement recovery while ensuring all lien obligations are properly resolved.
The Lien Negotiator will communicate directly with medical providers, health insurance carriers, lienholders, and internal departments to obtain balance confirmations, negotiate reductions, and prepare accurate lien breakdowns for settlement disbursement. This position requires strong negotiation skills, attention to detail, financial accuracy, and a thorough understanding of personal injury settlements and medical billing.

Responsibilities Include:
- Review case files, settlement information, medical bills, hosoital liens, and treatment records
- Obtain updated balances and final lien confirmations from medical providers and lienholders
- Negotiate reductions of medical bills, provider liens, letters of protection, and other outstanding balances
- Communicate with medical providers, billing departments, collection agencies, and health insurance carriers
- Identify and resolve discrepancies involving billing, payments, adjustments, and outstanding balances
- Prepare detailed and accurate lien breakdowns for settlement disbursement
- Reconcile negotiated balances against settlement proceeds
- Confirm that all negotiated reductions and agreements are documented in writing
- Maintain detailed notes, correspondence, and supporting documentation within the case management system
- Monitor assigned cases and follow up consistently to prevent delays in client disbursements
- Prioritize urgent and high-value files based on settlement and disbursement deadlines
- Identify disputed, excessive, or high-exposure liens requiring attorney or management review
- Collaborate with case managers, attorneys, accounting, and disbursement teams to resolve liens efficiently
- Ensure lien negotiations comply with firm procedures and settlement requirements
- Maintain accurate tracking reports and meet established weekly productivity goals
Requirements:
- Minimum of 1 years of experience in personal injury lien negotiations
- Direct experience negotiating medical liens and provider balances
- Strong understanding of personal injury settlements and lien resolution
- Ability to review medical bills, payment histories, and balance statements
- Experience preparing lien breakdowns and settlement allocations
- Strong mathematical, analytical, and reconciliation skills
- Excellent verbal and written communication skills
- Strong negotiation and conflict-resolution abilities
- Ability to manage a high-volume caseload and meet deadlines
- Ability to work independently while collaborating with attorneys and internal departments
Preferred Experience:
- Experience negotiating letters of protection and provider liens
- Familiarity with health insurance, Medicare, Medi-Cal, ERISA, or other reimbursement claims
- Experience working with lien resolution, accounting, or settlement disbursement departments
- Knowledge of medical billing terminology, adjustments, and payment ledgers
- Experience handling high-volume settlement pipelines
- Proficiency with case management systems such as Filevine or a similar platform
