Duties
- Assist residents or clients with personal care tasks such as bathing, grooming, dressing, and toileting to ensure comfort and hygiene.
- Support individuals with mobility using proper patient transfer methods and equipment like Hoyer lifts or manual handling techniques.
- Observe and document patient conditions, including vital signs, medication adherence, and behavioral changes, while maintaining HIPAA compliance to protect privacy.
- Prepare nutritious meals tailored to dietary needs and assist with feeding when necessary, promoting healthy eating habits.
- Administer medications following care plans and ensure accurate documentation of medication administration.
- Support residents during activities and outings by providing personal transportation assistance and ensuring safety at all times.
- Maintain a clean environment by performing light cleaning duties in accordance with health standards, fostering a safe living space.
- Collaborate with healthcare teams to update care plans, report concerns related to dementia or neurodevelopmental disorders like autism, and support memory or hospice care needs.
- Engage in patient observation to identify changes in condition or behavior that require escalation or intervention.
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
- Demonstrated experience providing patient personal care assistance, including ADLs such as bathing, dressing, grooming, and meal prep.
- Knowledge of patient monitoring techniques including vital signs measurement and medication administration.
- Familiarity with specialized care such as Alzheimer’s disease, dementia care, memory care, or hospice support is advantageous.
- Experience working with individuals with disabilities—developmental disabilities or intellectual disabilities—and neurodevelopmental disorders like autism enhances your ability to provide tailored support.
- Certification in CPR and First Aid is required; additional training in manual handling or use of assistive devices like Hoyer lifts is beneficial.
- Strong communication skills for client interaction and team collaboration are essential for delivering high-quality care.
