The Residential Lead Case Manager offers the opportunity to work in a dynamic, team oriented, residential treatment environment helping individuals along the path of long-term recovery. The Residential Case Manager will work as an integral part of a multi-functioning team that works interdependently with each other.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:
- Oversee casemanagers
- Provide assessment, referral, linking and monitoring for clients in the residential treatment setting.
- Complete case management assessments upon entry.
- Complete fall risk assessments and fall risk plans upon entry.
- Support clients in developing an unplanned discharge plan at entry.
- Assist Team Lead with resident discharges including ensuring that discharge checklist is completed.
- Engage clients in case management meetings on a regular, scheduled basis and document in the EHR.
- Maintain primary responsibility for assisting residents with completing SNAP and other applications.
- Attend weekly treatment team meetings and complete the required documentation in the EHR.
- Coordinate with clients on gaining access to a PCP and a physical, as well as any specialty care recommended.
- Coordinate transportation and educate clients on types of transportation available.
- Engage in discharge planning at the onset of treatment with an emphasis on income, housing plans, connections to care and natural supports.
- Coordinate care with each clients MCO coordinator.
- Maintain a high level of engagement with all residents to ensure an environment of recovery.
- Continually obtain ROIs as referrals are made and care is coordinated.
- Maintain a positive, professional attitude toward residents, staff, and volunteers.
- Plan and/or facilitate house group activities and maintain attendance sheet for all activities.
- Handle crisis situations in a calm supportive manner.
- Maintain confidentiality and comply with company, state, federal and HIPPA rules and regulations.
- Create a safe and healthy environment for residents.
- Other assigned duties.
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
- Bachelor’s degree in human services with 1 year of experience with clients experiencing SUD/co-occurring SUD/MH OR CSAC-A OR CSAC/CSAC-R
- Experience working in a residential care setting
- Valid driver's license
- Self-motivated
- Availability to work some evenings.
- Good communication and organizational skills
- Ability to maintain program requirements and boundaries with compassion and respect
- Problem solving abilities
- Ability to complete and submit documentation and reports
Job Type: Full-time
Pay: $22.00 - $27.00 per hour
Expected hours: 40 per week
