The W Group Assisted Living is seeking a compassionate and experienced Full-Time Registered Nurse (RN) to join our team in an Adult Care Facility/Assisted Living setting.
This is a great opportunity for an RN who enjoys working with the senior population, values teamwork, and is comfortable coordinating care with residents, families, physicians, third-party providers, and facility leadership.
The RN will help support daily clinical operations under the direction of the Director of Patient Services while ensuring high-quality resident care and compliance with applicable New York State regulations.

Responsibilities:
- Conduct resident assessments and complete required clinical documentation
- Complete UAS-NY assessments at least every 6 months or with a change in resident condition
- Develop and update resident Plans of Care every 6 months or as needed
- Coordinate care with physicians, case management, medical providers, therapy teams, medication teams, CHHAs, MLTC/PACE programs, and other third-party providers
- Evaluate hospitalized residents and assist with safe discharge planning and re-entry to the facility
- Monitor changes in resident condition and communicate updates appropriately
- Assist with medication reconciliation and care coordination with physicians
- Supervise and support HHA staff to ensure quality care and documentation compliance
- Assist with training and in-servicing staff as needed
- Maintain accurate resident records and complete incident reporting as required
- Monitor infection control and OSHA compliance
- Respond appropriately to emergencies and participate in emergency procedure drills
- Perform additional duties as assigned by the Director of Patient Services or Administrator
Requirements:
- Current and active New York State RN license required
- Associate’s Degree in Nursing required
- BLS Certification required
- Strong communication and interpersonal skills
- Ability to work professionally with residents, families, staff, and outside providers
- Experience in Adult Care, Assisted Living, ALP, LHCSA, SNF, LTC, or home care preferred
- UAS Certification preferred
- BSN preferred
- Knowledge of home care policies and procedures preferred
- Must be compassionate, dependable, organized, and team-oriented
