Job Summary
Join our team as a Community Outreach Worker and become the connector between Wolomi and the moms who need us most. As the first point of contact between Wolomi and prospective moms, you'll set the tone for their entire experience with our program, so a warm, trustworthy first impression is central to this role. In this flexible, PRN role, you will identify pregnant individuals, new mothers, and the organizations that serve them, building trusted relationships and referral pathways that bring moms into Wolomi's Mama Companion program. Your work will expand our reach into underserved communities and ensure more moms have access to compassionate, culturally sensitive perinatal support.
Duties
- Identify and build relationships with community organizations, clinics, and providers who serve pregnant individuals and new mothers, including OB clinics, WIC offices, community health centers, shelters, and social service agencies.
- Develop and maintain warm handoff pipelines that connect moms from partner organizations directly to Wolomi Mama Companions.
- Conduct direct outreach to pregnant individuals and new mothers in the community, at events, and through partner sites to introduce them to Wolomi's services.
- Serve as the first point of contact for prospective moms, making initial introductions warm, clear, and welcoming so moms feel comfortable engaging with Wolomi from the very first conversation.
- Represent Wolomi at community events, health fairs, and provider meetings to build visibility and trust.
- Track and document outreach activity, referral sources, and handoff outcomes using standardized tools.
- Collaborate with Mama Companions and Wolomi leadership to refine referral processes and close gaps in the pipeline.
- Maintain up-to-date knowledge of local resources, eligibility criteria, and referral requirements across partner organizations.
- Bachelor's degree required.
Qualifications
- Experience in community outreach, health navigation, case management, or relationship-based referral work.
- Established relationships with community organizations, clinics, or social service agencies in Solano County (preferred).
- Strong communication and relationship-building skills; comfortable representing an organization to external partners.
- Comfortable working independently with minimal day-to-day supervision, managing your own schedule and outreach targets.
- Self-starter who thrives in a flexible, startup environment.
- Demonstrated ability to work with individuals from underserved communities with sensitivity and cultural awareness
- Comfortable being the face of Wolomi in first-time interactions with moms, making a strong, trustworthy first impression matters more than a polished sales pitch.
- Bilingualism is a plus, especially Spanish or Tagalog, the two most common non-English languages spoken in Solano County.
- Proficiency in social media management tools and technical platforms for outreach purposes
- Excellent organizational skills combined with writing proficiency for reports and promotional materials
- Background in maternal health, doula work, or community health is a plus but not required.
