American Traveler is hiring an LPN/LVN with 1+ year of LTC/skilled nursing experience and active NY State license for a long-term care facility evening shift assignment.

State/government-run long-term care center offering rehabilitation therapies, skilled nursing, memory care, and wound care
-
300-bed facility with a patient ratio of 1:18-20

Alzheimer's and dementia patient population

Evening shift, 8 hours, 5 days per week (2:45 PM - 11:15 PM)

Every other weekend coverage required upon center request
-
PCC (Point Click Care) charting system used, though not required for consideration
-
Active NY State LPN/LVN license required at time of consideration; license required if applicable

Minimum 1 year of Long-Term Care (LTC) or skilled nursing experience required

Current BLS certification required

Must be able to pass a medication administration (med pass) test and clinical observation
-
Strong attendance and punctuality required
-
Ability to work in a fast-paced environment with flexibility for scheduling needs

Administer medications and treatments as prescribed and perform medication passes accurately and safely
-
Monitor and report changes in resident condition with timely and accurate documentation
-
Provide nursing care in accordance with facility policies and supervise CNAs on assigned units

Any solid color scrubs with closed toe shoes required

First-time travelers accepted

Local candidates accepted

Shift cannot be changed once offered

RTO must be approved at time of consideration; requests cannot be approved after offer, and every holiday cannot be requested off

Call-outs must be given with more than 24 hours notice

Start dates occur every 2 weeks due to orientation schedule, and orientation cannot be missed

Training modules must be completed no later than 3 days prior to assignment start date
