Duties
- Conduct detailed patient assessments, including vital signs, wound evaluations, and physiological observations using EMR/EHR systems.
- Assist nurse practitioner with wound treatments such as dressing changes, graphing and ostomy care.
- Assist with diagnostic evaluations, specimen collection/processing, and documentation of clinical findings in compliance with HIPAA.
- iCollaborate with multidisciplinary teams to develop and implement individualized care plans for patients with complex wounds or chronic conditions.
- Educate patients and caregivers on wound care techniques, infection control practices, nutrition, and pain management strategies to promote healing at home or in community settings.
- Maintain sterile processing standards during procedures such as suturing or sterile dressing changes while ensuring infection control policies are followed rigorously.
- Monitor patient progress through ongoing assessments, adjusting treatments as needed while documenting all interventions accurately within EMR/EHR systems.
Experience
- Proven experience working as an LPN in diverse clinical environments such as hospital medicine, skilled nursing facilities, outpatient clinics, or home & community care settings.
- Familiarity with EMR/EHR platforms like Epic, Cerner, Athenahealth, or eClinicalWorks to ensure seamless documentation and communication.
- Knowledge of ICD-9/10 coding systems for accurate medical documentation and billing processes.
- Experience working with individuals with disabilities or chronic conditions.

Pay: $23.00 - $30.00 per hour
