What You'll Be DoingLead Patient Care
- Perform in-home assessments and develop individualized care plans.
- Manage ongoing visits and clinical decision-making.
- Coordinate care across the healthcare continuum.
- Support patient transitions and recovery.
- Advocate for patients and families every step of the way.
Use Your Clinical Expertise
- Perform OASIS assessments and documentation.
- Manage chronic conditions and complex medical needs.
- Identify changes in condition and intervene proactively.
- Collaborate closely with physicians and interdisciplinary teams.
- Help reduce unnecessary hospitalizations and emergency room visits.
Build Relationships That Matter
- Become a trusted resource for patients and families.

What You BringRequired
- Active Minnesota RN license.
- Three or more years of nursing experience.
- Strong assessment and communication skills.
- Reliable transportation and valid driver's license.
- Confidence practicing independently.
Preferred
- Home health experience.
- OASIS experience.
- Previous case management experience.
