FIRST EPISODE PSYCHOSIS (FEP) SERVICE COORDINATOR
FMRS Health Systems, Inc., a private nonprofit Certified Community Behavioral Health Clinic (CCBHC), is seeking a First Episode Psychosis (FEP) Service Coordinator to lead and coordinate services for individuals experiencing a first episode of psychosis
The FEP Coordinator plays a critical leadership role in providing evidence-based early intervention services through a Coordinated Specialty Care (CSC) model. This position ensures comprehensive, recovery-oriented treatment that integrates behavioral health care, family education, supported employment/education, and medication management
The Coordinator will collaborate with psychiatrists, therapists, case managers, and community partners to ensure individuals experiencing early psychosis receive timely, person-centered services that promote recovery and long-term stability.
This position is based out of our Beckley Clinic and is generally Monday through Friday, 8am to 5pm but may require occasional evening or weekend work.
Essential Responsibilities
1. Facilitate the screening and referral of consumers likely to qualify for FEP.
2. Engage the assigned consumer and his/her family/support system while providing psychoeducation regarding early psychosis, as appropriate
3. Facilitate the development of an individualized, client-driven, strength-based FEP plan which includes crisis planning and is monitored and updated within established timeframes and as needed.
4. Provide case management and assist with supported education & supported employment services to assigned caseload.
5. Participate in weekly multidisciplinary team meetings to review assigned caseload.
6. Coordinate services and intervention with other FMRS services including, but not limited to, Wraparound, Mobile Crisis Response, Parent/Youth Peer Support, Therapy, Psychiatry, Community Engagement, etc.
7. Maintain schedule of activities in Carelogic (EMR), monitors alerts (failed activities, missing treatment plans, activities not statused, etc.) and resolve issues daily.
8. Track activities and complete monthly reporting within established timelines.
9. Provide high quality outreach activities to promote FMRS services in communities served.
10. Develop a network of community resources with key stakeholders to share and gain knowledge of available resources, identify gaps, and promote the development of resources.

REQUIREMENTS FOR POSITION:
· Bachelor’s degree in a Human Service related field required
· Prefer license and/or engaged in licensing process.
· Possess a valid WV driver’s license in good standing and access to a vehicle
·Experience working with individuals, teams and conducting community outreach is preferred.
·Applicant must be able to clear all background checks.

Physical Requirements:
1. Frequent communications, verbal and written.

3. Visually or otherwise identify, observe, and assess consumers.
4. Must be able to perform CPR & First Aid.
5. Must be able to perform Crisis intervention and de-escalation training (such as Right Response or equivalent) required within designated timeframe.
