Overview
We are seeking to hire experienced full-time Nurse Practitioners (NPs) and Physician Assistants (PAs) to join our growing healthcare team. You will work collaboratively with other healthcare providers in Skilled Nursing Facilities (SNFs) throughout the Philadelphia area to deliver high-quality, comprehensive care to patients.
Utilize your skills and expertise to diagnose, treat, and manage both short-term and long-term patients. The ideal candidate will have excellent communication skills and the ability to thrive in a fast-paced environment.
Duties
- Conduct thorough patient assessments and evaluations to determine health status and needs
- Develop and implement individualized treatment plans based on diagnostic findings
- Monitor patient responses to treatment and adjust plans as needed
- Ensure compliance with all healthcare regulations, including HIPAA
- Utilize electronic health record (EHR) systems for documentation and patient management
- Educate patients and families on health management and wellness strategies
- Collaborate with interdisciplinary teams to coordinate patient care
Skills
- Proficiency in diagnostic evaluation techniques
- Experience in medication administration and treatment planning
- Understanding of infectious disease protocols and geriatric care
- Ability to navigate EHR systems
- Strong communication and interpersonal skills
- Excellent organizational and time-management abilities
Requirements
- Authorized to work legally in the United States
- Licensed and Board Certified as a Nurse Practitioner or Physician Assistant in the state of Pennsylvania
- Prior experience in a Skilled Nursing Facility (SNF) or similar setting (preferred)
- Ability to work effectively in a team-based environment
- Strong work ethic and commitment to patient-centered care
- Fluent in English
- Quick learner with the ability to work efficiently
