Duties
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Observe changes in health status, reporting any concerns promptly
- Administer medications accurately following care plans and HIPAA regulations to ensure patient safety
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
- Use specialized equipment like Hoyer lifts or other assistive devices to safely transfer clients with mobility challenges
- Maintain detailed documentation of care provided, patient observations, and updates to care plans
- Support clients with cognitive impairments such as dementia or Alzheimer’s by implementing memory care strategies and engaging activities
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment in compliance with health standards
- Collaborate with healthcare professionals, social workers, and family members to coordinate comprehensive care
- Provide emotional support and companionship to promote mental well-being and social engagement
Skills
- Proven experience in home & community care, assisted living, or long-term care environments
- Knowledge of patient observation techniques, medication administration, and vital signs monitoring
- Strong understanding of Alzheimer’s disease, dementia care, autism spectrum disorder, and developmental disabilities
- Certified in CPR and First Aid; additional certifications like HIPAA compliance are preferred
- Ability to lift heavy objects safely using equipment such as Hoyer lifts when necessary
- Excellent communication skills for working with patients, families, and healthcare teams
- Experience working within resident’s personalized care plans while adhering to safety protocols
- Compassionate demeanor with a proactive attitude towards patient safety and well-being
