We are looking for a compassionate, organized, and relationship-focused Client Care Coordinator to join our leadership team and help ensure every client receives outstanding service.
Position Summary
The Client Care Coordinator serves as the primary liaison between clients, families, caregivers, and the office. This role is responsible for coordinating client services, supporting caregivers, conducting supervisory visits, monitoring quality of care, and helping maintain exceptional client satisfaction.

ResponsibilitiesClient Care & Quality Assurance
- Conduct new client assessments and routine supervisory visits
- Develop and update individualized care plans
- Monitor changes in client condition and communicate concerns appropriately
- Review caregiver documentation for accuracy and completeness
- Follow up on client concerns and service issues
- Maintain accurate documentation in WellSky
Family Communication
- Serve as the primary point of contact for assigned clients and families
- Build strong relationships through regular follow-up calls and visits
- Address concerns promptly and professionally
- Help families navigate changing care needs
Caregiver Support
- Assist with caregiver orientation and training
- Provide coaching and field support to caregivers
- Conduct quality assurance visits
- Recognize and encourage excellent caregiver performance
Care Coordination
- Coordinate services with hospice, home health, physicians, and other healthcare professionals as appropriate
- Help ensure client services are delivered safely and consistently
- Support client retention through proactive communication and problem-solving
Community Relationships
- Maintain positive relationships with referral partners, including hospice agencies, home health providers, hospitals, skilled nursing facilities, assisted living communities, and other healthcare professionals.
QualificationsRequired
- Minimum 3 years of experience in home care, home health, hospice, senior care, healthcare case management, or a related healthcare setting
- Excellent communication and customer service skills
- Strong organizational and time management abilities
- Ability to work independently and prioritize multiple responsibilities
- Valid California driver's license, reliable transportation, and proof of insurance
- Proficient with Microsoft Office and comfortable learning agency software
Preferred
- LVN license
- Experience with WellSky or similar home care software
- Supervisory or leadership experience
- Knowledge of California home care regulations
Schedule
- Monday–Friday
- Approximately 25-30 hours per week
- Combination of office work, client home visits, and occasional remote work as appropriate
- Occasional after-hours phone support for urgent client care matters. All authorized work performed outside scheduled hours will be compensated in accordance with California law.
