Job Summary
Vitalguard Homecare is seeking a North Carolina Licensed Registered Nurse (RN) to oversee and coordinate high-quality in-home care services. The RN will be responsible for clinical oversight, staff supervision, client assessments, care planning, and ongoing quality assurance for Medicaid and private-pay homecare services. This role ensures that all services are delivered safely, compliantly, and in alignment with each client’s individualized Plan of Care.
Key Responsibilities
Clinical Oversight & Client Care
- Conduct initial, ongoing, and reassessment visits to evaluate clients’ physical, emotional, and functional needs.
- Develop, implement, and update individualized Plans of Care in collaboration with clients, families, physicians, and care teams.
- Observe, assess, and document signs and symptoms, reporting changes in condition, adverse reactions to medications, or treatment concerns to the physician as required.
- Ensure care delivery aligns with physician orders, Medicaid guidelines, and agency policies.
Staff Supervision & Training
- Provide clinical supervision, consultation, and support to In-Home Aides, Companions, Sitters, and Respite Care Staff.
- Verify and document staff competencies through return demonstrations and direct observation.
- Conduct required 90-day and quarterly supervisory visits in clients’ homes, as applicable.
- Participate in hiring, onboarding, training, performance evaluations, and corrective action of homecare staff.
- Prepare written care instructions and assign staff appropriately based on client needs and staff qualifications.
Communication & Coordination
- Maintain regular communication with In-Home Aides and non-hands-on care staff regarding updates or changes in services.
- Serve as a clinical resource for staff during service hours, including availability for consultation and supervision.
- Participate in interdisciplinary collaboration to promote continuity of care.
Scheduling & Case Management
- Prepare, coordinate, and maintain schedules for client cases to ensure adequate coverage and continuity of care.
- Maintain accurate records of case assignments, supervisory visits, care plans, and staff documentation.
- Assist with managing on-call coverage according to agency policy to ensure appropriate clinical availability.
Quality Assurance & Program Evaluation
- Conduct selective program evaluations to identify service gaps, improve care delivery, and ensure goals are being met.
- Monitor service quality, client satisfaction, and staff performance; recommend improvements when deficiencies are identified.
- Support agency compliance with state regulations, Medicaid requirements, and internal policies.
Leadership & Administrative Support
- May serve as Supervisor for Companion, Sitter, and Respite Care Staff and may also function in an Agency Director or designee role, as needed.
- Assist leadership with operational planning, documentation standards, and continuous improvement initiatives.
Requirements & Qualifications
- Active North Carolina Registered Nurse (RN) license in good standing
- Minimum of two (2) professional references
- Proof of background check and consent for agency verification
- Copies of required medical and employee training records
- Proof of current tuberculosis testing (PPD or Mantoux) with annual updates thereafter
- Strong knowledge of homecare regulations, Medicaid services, and clinical supervision standards
- Excellent communication, organizational, and documentation skills

Pay: $50.00 - $80.00 per hour
