Position Summary:
The Homemaker Personal Care Assistant provides compassionate, person-centered support to individuals with intellectual and developmental disabilities in a home setting. This role focuses on companionship, promoting independence, and assisting with daily activities, including personal care.

Perform all delegated nursing services as outlined in the individual’s task analysis

Provide assistance with meals and personal care

Engage members in community activities and volunteering

Responsible for completing all required documentation

Responsible for following all programs as outlined in the member’s ISP and Behavior Support Plan.

Ensuring all services are provided in a timely and efficient manner in accordance with each individual’s ISP

Participate in team planning meetings as needed in conjunction with the Supportive Living Coordinator

Ensure the health and safety of all individuals

Maintain a clean and safe living environment

Assist with daily living tasks – grocery shopping, meal preparation, transportation to appointments, recreational activities, etc.

If necessary, fill in as an alternate at various worksites or other divisions

Uphold the rights, dignity, and respect of all members
-
Other duties as assigned
- Experience working with individuals with intellectual and developmental disabilities preferred

Compassionate, patient, and dependable

Ability to follow support plans and communicate clearly with team members

Ability to maintain confidentiality and professional boundaries

Must be at least 18 years old

Valid driver’s license and reliable transportation preferred

Ability to pass background checks and required screenings
-
Must remain in compliance with all regulatory standards governing program services, including appropriate trainings

Must have or obtain First Aid/CPR/AED training certification

Must be able to read, write, and understand English

Must be able to lift at least 50lbs on a regular basis

Must have the ability to be flexible with schedule, if needed

Ability to assist individuals with challenging behaviors through implementation of supported plans
