Choice Health at Home is seeking an experienced Home Health Business Development Liaison to drive referral growth and market expansion throughout West Broward County. The ideal candidate has a proven track record of generating home health referrals, developing relationships with physicians and facilities, and consistently achieving admissions goals.

Develop and maintain strategic relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and case managers

Grow market share by identifying and securing new referral sources

Execute territory development plans and achieve monthly admission targets

Serve as a trusted resource and clinical liaison for referral partners

Analyze referral trends and implement growth strategies

Collaborate with clinical leadership to ensure exceptional patient outcomes and customer satisfaction

Minimum 2-3 years of successful home health or hospice sales experience

Stable work history in the healthcare sales field

Demonstrated history of meeting or exceeding referral and admissions goals

Established healthcare referral relationships within Broward County strongly preferred

Strong knowledge of Medicare, home health regulations, and post-acute care services

Experience using CRM systems and managing a sales territory

HomeCare HomeBase experience preferred
