Job Summary
Join our dynamic healthcare team as a Occupational Therapist (OT) specializing in Home Health services! In this vital role, you will provide personalized, high-quality occupational therapy to patients in their homes, promoting independence and improving quality of life. Your expertise will support a diverse caseload, including pediatrics, geriatrics, individuals with disabilities, and those recovering from acute or post-acute care. This flexible position offers the opportunity to make a meaningful impact while working independently in a rewarding environment. Your skills in assisting with activities of daily living (ADLs), discharge planning, and case management will be essential to delivering comprehensive patient care that truly makes a difference.
Responsibilities
- Conduct thorough assessments of patients’ functional abilities and develop tailored care plans to meet their unique needs.
- Assist patients with activities of daily living such as dressing, grooming, feeding, and mobility to foster independence.
- Collaborate with interdisciplinary teams to coordinate discharge planning and ensure seamless transitions across care settings.
- Implement therapeutic interventions for diverse populations including pediatrics, geriatrics, individuals with autism or disabilities, and those in outpatient or skilled nursing facility settings.
- Educate patients and caregivers on adaptive techniques, safety measures, and home modifications to optimize daily functioning.
- Document patient progress accurately and maintain detailed records aligned with regulatory standards.
- Engage in case management activities to monitor patient outcomes and adjust treatment plans accordingly.
Qualifications
- Valid occupational therapy license or certification in the relevant state or region.
- Proven experience working with a variety of populations including pediatrics, geriatrics, ICU patients, or individuals with disabilities.
- Strong knowledge of physiology, anatomy, medical terminology, and therapeutic techniques applicable to home health settings.
- Experience assisting with ADLs, discharge planning, early intervention programs, and outpatient therapy is highly desirable.
- Familiarity with inpatient care environments such as hospitals or skilled nursing facilities is a plus.
- Ability to work independently while managing multiple cases effectively in a home health setting.
- Excellent communication skills for collaborating with patients, families, and healthcare teams.
