Position Title: Adult Day Training Program Coordinator
Location: Jacksonville, FL
Full/Part-time: Full-Time
Starting Hourly Rate: $17/hr
Signing Bonus: $700 ($250 paid at completion of 90-days & $250 at completion of 6-months)
R&R Loving Hands, Inc. Adult Day Training Program serves adults who enjoy being active community participants and being in the company of others. Consumers receive personalized attention and interaction with our staff, participate in weekly specialized programs, learn & develop life skills and attend a number of community outings each month.
We have an immediate opening for an Adult Day Training Program Coordinator. We are looking for an individual in the Jacksonville, FL area. The candidate will work in the office and/or in the community.
Minimum Requirements:
- Associate’s degree in Human Services or related field
- 3 years of supervisory/management experience
- 5 years of experience working with individuals with intellectual and developmental disabilities
- Knowledge of various types of developmental disabilities in an adult population
- Proficiency using MS Office (Word & Excel) programs or similar
- Effective communication skills
- Valid Florida driver’s license
- Pass a Level 2 Background screening
Responsibilities:
- Manages a caseload up to 25 consumers with intellectual and developmental disabilities.
- Supervises up to 10 direct care staff.
- Coordinates & supervises daily program operations to ensure the program is operating within regulatory standards for all governing bodies.
- Assigns staff responsibilities and monitors staff performance.
- Conducts a minimum of six (6) staff meetings per year.
- Documents incident reports where required.
- Participates in HR-related functions for ADT staff.
- Provides information about ADT program, provides site tours and supervises on-site consumer intake activities.
- Participates in development of Individual Implementation Plans and Training Data Forms for each consumer and oversees outcome measurements for all IIP goals and activities.
- Participates in annual support plan meetings.
- Creates & disseminates required reports monthly, annually and as needed.
- Ensures that all consumer meetings are scheduled and held.
- Oversees health and safety of consumers, staff and facility.
- Serves as on site liaison between coordinators of health services, transportation and facilities management as well as behavior services.
- Participates in program self-assessments, audits & surveys as directed by CEO/President.
- Provides training for staff; participates in development of staff training programs in conjunction with Human Resources.
- May be required to perform duties of program or transportation staff as required.
- Reviews staff documentation for attendance, notes, summaries and memos.
- Performs other functions as directed by CEO/President.
Starting pay rate: $17/hr depending on degree & level of experience.
