We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our evening shift team at Greendale Park Nursing & Rehab, a premier nursing and rehabilitation center located in Greendale, Wisconsin. As a Licensed Practical Nurse (LPN) on our team, you will have the opportunity to provide high-quality care to our residents while working collaboratively with our interdisciplinary team of healthcare professionals.
Responsibilities:
Provide direct patient care to our residents, including administering medications, wound care, and treatments as prescribed by physicians
Develop and implement individualized care plans to meet the unique needs of each resident
Monitor and report changes in resident condition to the registered nurse and/or physician
Maintain accurate and up-to-date records of resident care and treatment
Collaborate with other healthcare professionals to ensure seamless delivery of care
Participate in quality improvement initiatives to enhance the overall quality of care
Requirements:
Current LPN licensure in the State of Wisconsin
Current CPR certification
High school diploma or equivalent required; associate's or bachelor's degree in nursing or related field preferred
Minimum 1 year of experience as an LPN in a long-term care setting preferred
