About Hubbert Homes
Hubbert Homes is a nonprofit transitional housing and sober living organization serving Alameda County and Santa Clara County. Its services include housing-linked support for adults with complex medical, behavioral, and social needs, including Enhanced Care Management and other CalAIM-aligned services coordinated with referral partners, hospitals, managed care plans, and community organizations.
Position Summary
The Community Health Worker supports clients through appointment coordination, referrals, service navigation, and ongoing follow-up. This role helps clients access medical care, behavioral health care, recovery services, housing supports, benefits, and community resources, while strengthening continuity between Hubbert Homes and the larger care team.
This version is intended for candidates who already hold a Community Health Worker certificate of completion from a qualifying training program. DHCS states that for Medi-Cal CHW services, CHWs may qualify through the certificate pathway, and the supervising provider is responsible for determining whether the certificate meets Medi-Cal policy requirements.
Key Responsibilities
- Coordinate appointments and referrals for medical, behavioral health, recovery, and social service needs
- Help clients navigate provider systems, complete next steps, and reduce missed appointments
- Provide outreach, engagement, reminder calls, and post-appointment follow-up
- Support intake, care planning, and coordination with internal staff and outside providers
- Document services, referrals, and client progress in a timely manner
- Maintain professional communication with referral partners, care teams, and Hubbert Homes leadership
- Assist with client education, resource navigation, and problem-solving barriers to care
- Deliver a mix of field-based, on-site, phone, and telehealth support as appropriate
Minimum Qualifications
- High school diploma or GED
- Current Community Health Worker certificate of completion from a recognized or qualifying CHW training program
- Experience supporting clients with appointments, referrals, service coordination, navigation, outreach, or related work
- Strong written and verbal communication skills
- Ability to work independently while maintaining strong follow-through and documentation
- Comfort working with clients experiencing complex social, medical, and behavioral health challenges
Desired, Not Required
- Experience with an EHR system
- Experience writing case notes, intake assessments, and care plans
- Experience in Medi-Cal, ECM, Community Supports, or related care coordination settings
- Experience in housing, behavioral health, reentry, or recovery-focused programs
- Bilingual ability
- Familiarity with Alameda County and Santa Clara County health and social service systems
Pay: $20.00 - $26.00 per hour
License/Certification:
- CHW certification (Preferred)
