Job Summary: As an APRN, you will provide high-quality, patient-centered care, working under the supervision of a licensed physician in accordance with Tennessee state law. You will utilize your advanced clinical expertise to manage a variety of healthcare needs, collaborate with our multidisciplinary team, and ensure that patients receive the best possible care in an efficient, timely manner.
Key Responsibilities:
Comprehensive Patient Care:
Initial Assessments & Medical History: Conduct comprehensive health assessments, including thorough patient histories, physical exams, and the review of diagnostic test results to formulate appropriate care plans.
Diagnosis & Treatment: Diagnose acute and chronic conditions, develop individualized treatment plans, and provide evidence-based medical interventions. You will manage common medical conditions such as hypertension, diabetes, asthma, and more.
Follow-Up & Monitoring: Monitor patient progress and modify care plans based on clinical judgment, laboratory results, and patient feedback. Ensure all follow-up appointments and communications are scheduled and documented appropriately.
Prescribing & Medication Management: Prescribe Medications: Prescribe appropriate medications, including controlled substances, in line with the state’s supervisory guidelines and organizational policies.
Medication Management: Provide medication education, monitor for side effects, manage refills, and coordinate with the pharmacy to ensure that patients’ medications are up to date and effective.
Collaboration with Supervising Physician: Collaborate with the supervising physician for treatment strategies and adjustments, particularly in complex cases or those requiring specialized care.
Patient Education and Counseling: Patient Education: Offer counseling on disease prevention, health promotion, and self management of chronic diseases. Explain diagnoses, treatment options, medication regimens, and preventative measures to patients in an understandable manner.
Health Coaching: Guide patients in making lifestyle changes, including nutrition, physical activity, smoking cessation, and stress management.
Family Education: Educate families about their loved ones' health conditions, treatment plans, and follow-up care needs.
Clinical Procedures and Care:
In-Office Procedures: Perform minor office-based procedures such as wound care, suturing, joint injections, and the removal of skin lesions. Additionally, assist in more complex procedures under the guidance of the supervising physician.
Technology and Diagnostics: Order and interpret diagnostic tests, including lab work, radiology studies, and other diagnostic imaging. Ensure timely follow-up on test results and adjust treatment plans accordingly.
Chronic Disease Management: Manage care for patients with chronic conditions, such as diabetes, COPD, hypertension, and more. Ensure consistent monitoring and patient education on disease management.
Coordination and Communication:
Multidisciplinary Collaboration: Work closely with physicians, specialists, nurses, medical assistants, and other healthcare professionals to coordinate care and optimize patient outcomes.
Patient and Family Advocacy: Serve as an advocate for patient needs, addressing questions, concerns, and ensuring that the patient’s preferences are considered in their treatment plan.
Supervision and Support: Provide guidance and mentorship to other clinical staff, such as medical assistants or junior providers, to ensure high standards of care and patient satisfaction.
Administrative Responsibilities: Medical Documentation: Maintain accurate, up-to-date patient records in the Electronic Health Record (EHR), ensuring compliance with all regulatory requirements, including HIPAA.
Quality Improvement: Participate in practice-based quality improvement initiatives, including improving patient care processes, reducing wait times, and ensuring compliance with healthcare guidelines.
Billing and Coding: Ensure that services provided are correctly documented and coded for billing purposes.
Stay Current: Keep abreast of the latest clinical research, treatment protocols, and medical technologies to maintain cutting-edge practice.
Professional Growth: Engage in continuing education, attend professional development seminars, and participate in healthcare conferences to enhance knowledge and practice skills.
Qualifications:
Education and Licensure:
Master’s or Doctoral degree in Nursing (MSN or DNP) from an accredited nursing program. Current, active APRN license in the state of Tennessee.
National certification as a Nurse Practitioner (NP), or another advanced practice role in the area of Family Practice, Pediatrics, Adult-Gerontology, etc.
Active certification in Basic Life Support (BLS).
Experience:
Minimum of 2 years of clinical experience as an APRN in a primary care, urgent care, or specialty setting, with strong emphasis on family medicine, internal medicine, or pediatrics.
Experience working within a collaborative practice model with supervising physicians and interdisciplinary teams.
Familiarity with Tennessee state regulations regarding supervisory agreements and APRN practice.
Skills and Abilities:
Clinical Expertise: Demonstrated proficiency in diagnosing, treating, and managing a wide range of medical conditions, both acute and chronic.
Clinical Procedures: Competence in performing clinical procedures such as suturing, joint injections, and wound care, preferred.
Patient-Centered Care: Strong ability to build rapport with patients and families, ensuring a high level of satisfaction and trust.
Documentation and Compliance: Skilled in using Electronic Health Records (EHR) and maintaining documentation standards, including for billing and coding.
Team Collaboration: Ability to work effectively in a collaborative team environment, providing mentorship and support to junior staff members.
