Responsibilities
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patient health by observing vital signs, reporting changes, and maintaining accurate care records in compliance with HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote proper nutrition
- Administer medications following prescribed schedules and document all medication-related activities accurately
- Support residents with cognitive impairments such as dementia or Alzheimer’s disease through engaging activities and memory care techniques
- Maintain a clean and safe environment by performing light cleaning duties, laundry, and sanitation of common areas in line with health standards
- Collaborate with healthcare teams to develop and implement personalized care plans that address each resident’s unique needs
Qualifications
- Proven experience in home health, assisted living, nursing home, or group home settings preferred
- Knowledge of patient observation techniques, vital signs measurement, and medication administration
- Certifications such as CPR and First Aid are highly desirable; ongoing training encouraged
- Experience working with individuals with developmental disabilities, autism spectrum disorder, or memory care needs is a plus
- Ability to lift heavy objects safely and perform physically demanding tasks as required for resident support
- Familiarity with HIPAA regulations to ensure confidentiality of patient information
- Compassionate attitude with excellent communication skills to foster trusting relationships with residents and team members
